Parent Consent Form 2020

The SCHA will permit Junior Players (14-18 Years of Age as of 31st December in the year the Competition commences) to play in the Seniors Competitions providing parents provide written consent. It should be noted that senior games are very competitive and any junior considering playing should be a competent player and capable of playing at the required level, including the physicality of the game.

By completing the form below, and submitting on-line, you are hereby agreeing to allow your son/daughter to play in the senior competitions. You also acknowledge and assume responsibility for the risk that may come with playing in the senior competitions.

It is the right of the individual Club, as well as the SCHA, to deny permission for a junior player to participate into the senior competition if considered not suitable or appropriate.
